Please print and complete each required form. Explanations for each form are listed below. This completed paperwork must be submitted at your induction appointment.

Explanation of Required Forms
- New Employee Information Form: You must complete this form.
- Notification and Authorization to Conduct Employment Background Investigation: Cranberry Township requires all newly hired employees to have a Pennsylvania Criminal Background Check conducted by the State Police (PSP clearance) to ensure negligent hiring does not occur. This form must be completed in order to perform this background check. This form will also provide the Township with authority to conduct a Driver’s Background Check for all employees who will drive a Township vehicle.
- Fair Credit Reporting Act Disclosure: This form is also required in order for the Township to process your PA State Police clearance and Driver’s License Background check (if applicable). Please Review your Summary of Rights prior to completing this form.
- W-4: You must complete page 1 so that the proper federal taxes can be withheld from your paycheck. Pages 2-4 are for your own use for determining your personal allowances. Please read all instructions before completing the form.
- Local Service Tax Exemption: If you would like to file for an exemption the Local Services Tax, you must submit this form. If you have another employer withholding the tax, you must also submit a copy of your last pay stub from the current year or a copy of your W-2 from last year showing it is being withheld. Or if this is your first job and want to file for the exemption because you will be making less than $12,000 this year, you must indicate “FIRST JOB” on the front of the Tax-Exemption Form.
- Authorization for Direct Deposit: Complete this form to enable direct deposit of your paycheck into your bank account. A voided check must accompany this form. If you plan to have your paycheck deposited into a savings account, please provide your bank information, including the routing and account number.
- Local Earned Income Tax Residency Certification: This form is used to report essential information for the collection and distribution of Local Earned Income Taxes.
- Emergency Contact Form: This form must be completed if you work in the Parks & Rec Department (the Waterpark, Camp Cranberry, Kids Club, Preschool, or any other position in Parks & Rec)
Additional Required Forms for Minors (under Age 18)
All minors under the age of 18 – You must provide a copy of your Work Permit at your induction appointment. If you do not yet have a work permit, you will need to obtain one from your school. The application can take several weeks to be processed, so please apply for your permit as soon as possible. Refer to: Instructions for Applying for a Work Permit. If you are under the age of 16, you will need to complete an additional form – Print, sign, and have your parent/legal guardian sign the following Acknowledgement form, and bring it to your induction appointment:
Additional Requirements for I-9 Employment Eligibility Verification (required for ALL employees)
A Form I-9 (Employment Eligibility Verification) has been provided to you via email. Please be sure to follow the instructions included in the email for completing the I-9 Form.
You will need to bring this completed form to your induction appointment. You will also be required to bring proper identification with you to satisfy the I-9 requirements (Examples: Passport, Driver’s License, Social Security Card, etc.).
Please review the attached I-9 List of Acceptable Documents to determine which forms of acceptable identification you wish to bring.
Your start date may be postponed if you do not have appropriate identification with you, so please make sure you review this document thoroughly. Please note the following regarding this requirement:
- We must see the ORIGINALS of your documentation. Copies are not acceptable.
- If you have something from List A (i.e., a Passport), you need only one form of identification.
- If you do not have a document from List A, you will need two forms of identification (one from List B AND another from List C). We cannot accept two forms of identification from the same column.
- All identification with an expiration date must be unexpired. If you are using a school ID for the I-9 requirements, it must be current.
